Advancing Community Wellness
Located on the second floor of the Town Center, the Assessment Center in partnership with PMLGH, will aim to advance community wellness through education and the prevention of neurodegenerative disease. Extending support beyond the Willow Valley Communities, the Assessment Center will be open to the public. Professionals from the interdisciplinary team of neurologists, neuropsychologists, geriatricians, executives, and researchers will advance efforts in research, awareness and access to neurological care.
An initial evaluation to establish a baseline, coupled with routine follow-up assessments with the practitioner, will be essential to developing a personalized approach for each patient. These ongoing check-ins provide the opportunity to re-evaluate the individual maintenance or treatment plan, while also potentially preventing hospitalizations, falls, and unnecessary caregiver stress.

- Assessment: offering convenient access, enabling earlier identification and connections to services, sooner
- Treatment: specializing in individualized care, the latest in breakthroughs, and the best practices for memory care
- Prevention: delivering the tools needed to promote a brain-healthy lifestyle, warding off cognitive decline
- Education: enhancing public awareness, eliminating stigma, improving well-being, and reducing caregiver stress
- Support: providing a hub for everyone impacted by memory loss, elevating quality of life for an entire community
- Research: advancing knowledge of risk factors, available treatments, preventive measures, and neurological care
- Trials: making trials more widely available, accessible, and simplified for those who desire to participate
The PMLGH Assessment Center will also host a blood biobank where Residents, guests, and patients can voluntarily participate and contribute to ground-breaking research studies.
